The Naples to Milazzo ferry times detail the route from the capital city of Campania, directly south to ferry the port on the Sicilian mainland. Available only at certain times of the year and only on certain days of the week, the crossings are operated by Siremar. 

This is a particularly long crossing, leaving Naples at 8 pm and arriving in Milazzo just after 2pm the next day; some 18 hours.
